Botulinum toxin is a clinically proven treatment for the prevention of chronic migraine, defined as 15 or more headache days per month, of which at least 8 have migraine features. Approved by NICE and widely used in NHS specialist headache clinics, it is considered a first-line preventive treatment for patients where other medications have not provided adequate relief.
The treatment involves multiple small injections across the head, neck, and shoulders, targeting specific muscle groups involved in triggering migraines. It does not cure migraines but significantly reduces their frequency and severity for most patients.

Is it right for you?
Adults diagnosed with chronic migraine (15 or more headache days per month, with at least 8 having migraine features) who have not achieved adequate relief from other preventive medications.
Please tell us at consultation
Not suitable during pregnancy or breastfeeding. Not recommended for episodic migraine (fewer than 15 headache days per month), as this does not meet the clinical criteria for this treatment. Not suitable for patients with neuromuscular conditions or known allergies to botulinum toxin. A full medical assessment is carried out at your consultation.

Does it actually work for migraines?
Yes. Botulinum toxin for chronic migraine is NICE-approved and has been validated in large-scale clinical trials (the PREEMPT trials). It is an established, evidence-based preventive treatment, not a cosmetic use.
How many treatments before I see a difference?
Most patients begin to notice improvement within 4–6 weeks of the first cycle. The full benefit typically becomes apparent after 2–3 treatment cycles, and results continue to improve with each successive treatment.
Do I need a GP referral?
No referral is required. However, a thorough clinical consultation is required before treatment to assess your diagnosis, headache frequency, and previous treatments.
Is this the same as cosmetic Botox?
The substance (botulinum toxin) is the same, but the technique, dosage, and injection sites are entirely different. Chronic migraine prevention uses the PREEMPT protocol, a clinically validated approach specific to headache prevention.
What if it doesn’t work for me?
Not all patients respond equally. We recommend a minimum of 2 treatment cycles before assessing response. If there is no meaningful improvement after 2 cycles, we will discuss alternative options with you.
